Dementia Care
Bluegrass Dementia Care is a new program that imagines a future where those with dementia and their caregivers receive seamless, compassionate care.
Bluegrass Care Navigators is participating in The Guiding an Improved Dementia Experience Model (GUIDE) and Bluegrass Dementia Care is provided through that program.
Bluegrass Dementia Care provides a standard approach that
- provides comprehensive care coordination and caregiver education,
- improves the quality of life for people living with dementia and their caregivers,
- reduces strain on their unpaid caregivers, and
- offers a 24/7 support line
With support from Bluegrass Dementia Care people living with dementia can remain safely in their homes and communities.
Bluegrass Dementia Care Criteria:
- Diagnosis of mild, moderate, or severe dementia
- Enrollment in both Medicare Parts A & B, with Medicare as the primary payer
*Exclusions include enrollment in Medicare Advantage or other Medicare health plans, Medicare hospice benefit, long-term nursing home residency, alignment with another GUIDE Model Participant, and non-residency in the Bluegrass Dementia Care service area.
Bluegrass Dementia Care Participants Receive :
- A comprehensive telehealth assessment by a Nurse Practitioner
- A Care Navigator for personalized care and support
- Person-centered care planning
- Ongoing monitoring, care coordination, and medication management
- Referrals to community-based services and caregiver support groups
- Caregiver education, skills training, and dementia diagnosis information
- Coordination of respite care
- 24/7 support
