Bright Path Program Improves Patient’s Quality of Life

Facing a serious illness in the family is undeniably challenging, especially when it involves your child. Bright Path, a pediatric palliative care program offered by Bluegrass Care Navigators, provides tailored medical assistance for medically fragile infants, children, adolescents and young adults living with serious illnesses to improve their quality of life through enhanced resources, education and support. 

As a nurse case manager for Bright Path patients, Lauren Samuels (LPN) plays a vital role in supporting families facing serious illness. Samuels and her co-workers provide home visits that offer personalized care and connect families with essential resources. Additionally, Bright Path staff collaborate with other healthcare providers to effectively manage symptoms and ensure comprehensive support for both patients and parents. 


“As a nurse within Bright Path, I'm committed to understanding the distinct challenges each family faces,” Samuels said. “I provide personalized education and care coordination, advocating tirelessly for single-income or single-parent households, as well as grandparents and great-grandparents raising children. By being a voice and advocate for our patients, I aim to elevate the quality of life for every child in our care, offering a beacon of hope amidst their journey.” 


Katie* is one of many who have had a transformative experience because of the care they received through Bright Path. Diagnosed with a genetic disorder, Katie struggled with severe pain, cognitive disabilities, and other symptoms associated with her disease. With the support of Bright Path, Katie now attends a day care program where she has found companionship and receives additional resources through a waiver program. With the guidance of her nurse practitioner, her pain is effectively managed, allowing her newfound energy to pursue activities she enjoys. Recently, during a home visit, her nurse noticed that the atmosphere was vibrant, with Katie and her family actively participating in crafting activities—a stark contrast to previous encounters. 


Bright Path specializes in managing symptoms associated with various pediatric illnesses and chronic conditions, such as progressive cancer, neuromuscular disorders, genetic or congenital diseases, neurodegenerative disorders, cardiovascular conditions, and pulmonary disorders. 


“Hope is hard to define sometimes,” Samuels said. “While we've seen significant success with many in our Bright Path program, it's vital to recognize that success looks different for each family. Bright Path guides families to discover their individual path to hope, and Katie’s story reminds me that I’m privileged to be part of this amazing journey every day as I watched her make remarkable progress and improve her quality of life despite her diagnosis.” 


Bright Path's effectiveness stems from our direct engagement with patients in their homes, transcending the role of mere healthcare providers to become allies alongside families. We advocate for essential services and facilitate access to necessary resources, equipment, and treatments vital for our young patients' well-being. Enhancing communication among various specialists fosters a cohesive support network, enveloping each child and family in a seamless web of care.

From therapy dog visits with Lilly to sewing masks during the pandemic, Paige has consistently found ways to meet others’ needs and give back to her community. Alice, a young bereavement client, experienced profound loss at a very early age. Alice herself had been diagnosed with leukemia at just 2 1/12 years old and underwent more than two years of treatment before her cancer went into remission. Then, at the age of 10, Alice’s mother passed away under the compassionate care of Bluegrass Hospice Care after her own prolonged battle with cancer.
