Bluegrass PACE Care provides all needed medical and supportive services.
Bluegrass PACE Care services include:
- Preventative care
- Support managing symptoms of chronic disease
- Primary care including physician and recreational services
- Daily activities of living including bathing, dressing and eating
- Provider network of medical specialists
- Nutritional support, breakfast and lunch provided at the Bluegrass PACE Center
- Occupational and physical therapy
- Dentistry
- Hospital and emergency care support and management
- Prescription drugs and medication management
- Mental wellbeing through counseling and social work support
- Social services including caregiver support and respite care
- Transportation to and from the PACE Center and all authorized medical appointments
