Most patients who move from the hospital to their home are in poor health or recovering from an injury or illness. They have a safe environment and support from family members or caregivers. Transitioning from the hospital into the community, however, can be extremely difficult for those experiencing homelessness. Our Medical Respite program provides an environment to safely recuperate after hospital discharge for patients experiencing homelessness. It also allows patients to connect to permanent housing options, reunite with their families, and receive referrals to ongoing support services.
For some hospice patients, there can be a lack of personal resources to obtain even basic comfort items, like toothpaste, socks, and lotion. You can help fill the gap for these patients by donating items to stock our resource closets, so that when we have a patient in need, we can provide them with the item to help bring them comfort and dignity on their end-of-life journey with us.
Our integrative medicine program coordinates complementary therapies into our care plan for patients and families. With a focus on what is most important to our patients and the therapies that may be most helpful, our integrative medicine team works with each care team to support patients with practices such as massage, music and art therapy, and animal-assisted therapy.
PACE stands for “Program of All-Inclusive Care for the Elderly.” With Bluegrass PACE Care, our experts connect participants with all aspects of their care and support their needs, including everyday tasks. We deliver care at home or at the PACE Center, providing comprehensive health care services and helps a person live in the community safely.
Some hospice patients need to be cared for in our Hospice Care Centers or in an inpatient setting rather than in their own homes. Friends and family members often spend many hours at their loved one's bedside. Our Carts offer items to make their stay more comfortable.
Bright Path is a pediatric palliative care program that provides specialized medical care for infants, children, adolescents, and young adults who are living with a serious illness. This program is designed to help manage symptoms for many pediatric illnesses and chronic conditions, including progressive cancer, neuromuscular disorders, genetic or congenital diseases, neurodegenerative disorders, cardiovascular conditions, and pulmonary disorders.
